  • TB test cost: $65 (skin test) at CityHealth San Leandro — no appointment needed

  • Visits required: 2 visits for skin test, 1 visit for blood test

What Is a TB Test?

A tuberculosis (TB) test checks for infection with Mycobacterium tuberculosis, the bacteria that causes TB. Importantly, most people who test positive for TB have latent TB infection (LTBI) — meaning the bacteria is dormant in their body and they’re not contagious and have no symptoms. A positive TB test doesn’t mean you have active TB disease.

TB testing is required for many purposes in the Bay Area and California: school enrollment, healthcare worker clearance, immigration medical exams, employment in healthcare, childcare, or education, and travel to certain countries.

Types of TB Tests Available at CityHealth

Tuberculin Skin Test (TST) — Mantoux / PPD

The traditional TB skin test involves:

  1. Visit 1: A small amount of tuberculin (purified protein derivative, PPD) is injected just under the skin of the forearm. A small bubble appears, which is normal. This takes about 2 minutes.
  2. Wait 48–72 hours. Do not miss this window.
  3. Visit 2 (read appointment): A provider measures the induration (raised, hardened area) at the injection site — NOT redness. Size of the induration determines the result.

TST reading criteria:

  • ≥ 5mm induration: Positive for HIV-positive patients, recent TB contacts, patients with fibrotic changes on chest X-ray, immunosuppressed patients
  • ≥ 10mm induration: Positive for foreign-born individuals from high-prevalence countries, recent immigrants (<5 years), IV drug users, healthcare workers, children under 4
  • ≥ 15mm induration: Positive for all other individuals

Interferon-Gamma Release Assay (IGRA) — Blood TB Test

The QuantiFERON-TB Gold and T-SPOT.TB are blood tests that also detect TB infection. Advantages over TST:

  • Only requires one visit (blood draw, results in 24–48 hours)
  • Not affected by prior BCG vaccination (BCG vaccine is given in many countries and commonly causes false-positive TST results)
  • No need to return for a second reading appointment

IGRA is preferred for people who received BCG vaccine (common in Mexico, India, China, Korea, many other countries), people unlikely to return for TST reading, and healthcare workers.

Which TB Test Do I Need?

It depends on your purpose:

  • School/childcare enrollment: TST (PPD) is accepted by California schools; some require documentation of reading date and result
  • Healthcare worker clearance: Both TST and IGRA are accepted; IGRA preferred for BCG-vaccinated workers to avoid false positives
  • Immigration (I-693 civil surgeon exam): IGRA (QuantiFERON) required by USCIS; TST is no longer accepted for immigration medicals
  • Employment (non-healthcare): TST or IGRA depending on employer requirement
  • Travel medicine: Baseline testing before travel to high-burden countries; follow-up testing after return
  • Uncertain: Ask your provider or call CityHealth — we can advise based on your specific situation

How Long Does the TB Test Take?

Quick answer: A TB skin test (TST/Mantoux) requires two visits — a 5-minute placement on Day 1, then a reading visit 48–72 hours later. An IGRA blood test takes just one visit with results in 24–48 hours.

  • TST (skin test): Day 1 placement: 5–10 minutes. Return in 48–72 hours for reading. You must return within this window — results cannot be read before 48 hours or after 72 hours.
  • IGRA (blood test): Blood draw at one visit; results in 24–48 hours via patient portal

What If My TB Test Is Positive?

A positive TB test is not a crisis — it’s information. Next steps depend on your situation:

  1. Chest X-ray — to distinguish latent TB infection from active TB disease. CityHealth has on-site X-ray. Active TB shows characteristic findings on imaging; latent TB is usually normal.
  2. Symptom review — active TB causes cough lasting 3+ weeks, night sweats, unintentional weight loss, fever, fatigue. Latent TB has NO symptoms.
  3. If latent TB: Treatment with a short course of preventive antibiotics (9 months of isoniazid, or 3 months of INH + rifapentine, or 4 months of rifampin). This reduces the risk of progression to active TB by 60–90%. Treatment is voluntary but strongly recommended.
  4. If active TB: Referral to Alameda County TB Control Program for mandatory treatment (4 drugs for 2 months, then 2 drugs for 4 months). TB is a reportable disease in California.

TB Testing in Oakland: Why It’s Commonly Needed

Oakland and the broader Bay Area have higher TB testing demand than most US cities, for several reasons:

  • Large immigrant populations from TB-endemic countries (Southeast Asia, Latin America, sub-Saharan Africa)
  • High concentration of healthcare workers, school employees, and social service workers who require annual or baseline testing
  • Alameda County has one of the higher TB rates among California counties
  • Immigration medical exams (I-693) require TB testing for green card applicants

How Much Does a TB Test Cost at CityHealth?

CityHealth’s TB skin test (TST/Mantoux) costs $65 — with no appointment needed. Results are available within 72 hours (you return 48–72 hours after the test for the provider to read the results). This is an affordable, same-day option compared to calling ahead for a primary care appointment or waiting at a health department clinic.

Pricing summary:

  • TB Skin Test (TST/Mantoux): $65 — self-pay price
  • IGRA Blood Test (QuantiFERON-TB Gold): Billed to insurance (most accepted); call for self-pay pricing
  • Results: 48–72 hours for TST; 3–7 business days for blood test
  • Documentation: Official clearance documentation provided

Most major insurance plans cover TB testing as a preventive service at no cost to you. CityHealth accepts Blue Shield, Anthem, Alameda Alliance, Medi-Cal, and most other plans. If you’re paying out-of-pocket, the $65 self-pay rate is our standard flat rate — no hidden fees.
