COVID-19 Treatment in Oakland: Paxlovid and Antiviral Options

Testing positive for COVID-19 can feel overwhelming — but fast treatment makes a real difference. CityHealth provides same-day COVID treatment in Oakland, including Paxlovid and other antiviral options. If you’re looking for COVID treatment in Oakland within the critical treatment window, CityHealth San Leandro is ready to help.

COVID-19 Treatment Options Available Now

Paxlovid (Nirmatrelvir/Ritonavir) — The Primary Option

Paxlovid is a 5-day oral antiviral treatment for COVID-19 that reduces the risk of hospitalization and death in high-risk adults by 89% when started early. It consists of two drugs taken together:

  • Nirmatrelvir — inhibits the virus’s ability to replicate
  • Ritonavir — boosts nirmatrelvir levels by slowing its breakdown

Must be started within 5 days of symptom onset. Sooner is better — day 1 or 2 gives the best results.

Paxlovid is authorized for adults and children 12+ (≥40 kg) who test positive for COVID-19 AND are at high risk for severe disease. High-risk criteria include:

  • Age 65 or older
  • Immunocompromised (HIV, organ transplant, cancer on chemotherapy, immunosuppressant drugs)
  • Unvaccinated or not up-to-date on COVID vaccines
  • Obesity (BMI ≥30)
  • Diabetes
  • Heart disease, chronic kidney disease, chronic lung disease
  • Pregnancy
  • Down syndrome or certain neurological conditions

Important: Paxlovid has significant drug interactions. Ritonavir interacts with a long list of medications — statins, blood thinners (some), antidepressants, and more. This is why a provider visit is essential before prescribing. CityHealth providers perform a medication review before prescribing Paxlovid.

Lagevrio (Molnupiravir)

An alternative antiviral pill for adults 18+ who can’t take Paxlovid (due to drug interactions or contraindications). Less effective than Paxlovid but an option when Paxlovid isn’t appropriate. Also started within 5 days of symptoms.

Remdesivir (Veklury)

An IV antiviral administered in a clinical setting. Used for outpatients at high risk who can’t take oral antivirals. Requires 3 daily infusions. CityHealth can refer to infusion centers when appropriate.

Who Should Come to Urgent Care for COVID Treatment?

Come to CityHealth urgent care if you:

  • Just tested positive (home test or clinical test) and have risk factors for severe disease — come in within 24–48 hours of a positive test
  • Have symptoms for less than 5 days — you’re still within the Paxlovid treatment window
  • Have a positive home test but want clinical confirmation and a prescription in one visit
  • Are 65 or older and tested positive — your age alone qualifies you for treatment
  • Are immunocompromised for any reason
  • Have worsening symptoms — increasing shortness of breath, chest pain, confusion, symptoms not improving after 5 days
  • Want to know your options even if you’re lower risk — your provider will assess your individual situation

What Happens at Your COVID Urgent Care Visit?

  1. Testing — if you haven’t tested yet, we can confirm with a rapid antigen test on-site
  2. Clinical assessment — symptom review, oxygen saturation, risk factor evaluation
  3. Medication review — essential for Paxlovid; we check your current medications for interactions
  4. Prescription — Paxlovid or Molnupiravir sent to your pharmacy, or dispensed directly when available
  5. Monitoring plan — return precautions, when to escalate to ER, telehealth follow-up option

COVID-19 Symptom Severity: What Level of Care?

  • Mild symptoms (no breathing difficulty, can manage at home): Urgent care for antiviral evaluation if risk factors present; home management otherwise
  • Moderate symptoms (notable fatigue, significant cough, low-grade fever, oxygen 94–96%): Urgent care for evaluation, pulse oximetry, possible antivirals and supportive care
  • Severe symptoms (oxygen below 94%, significant shortness of breath at rest, confusion, chest pain): Emergency room — not urgent care

Paxlovid Rebound: What It Is and What to Do

A small percentage of people who take Paxlovid experience “Paxlovid rebound” — COVID symptoms return (or worsen) 2–8 days after completing the 5-day course. If this happens:

  • Test again — if positive, this is rebound (not reinfection)
  • Isolate again as you would with a new positive test
  • For most people, rebound is mild and resolves without additional treatment
  • If severe symptoms develop during rebound, come back in

COVID Care at Home: What Actually Helps

  • Rest — allow your immune system to work
  • Hydration — fluids are critical; fever and sweating cause significant fluid loss
  • Acetaminophen or ibuprofen — for fever and body aches
  • Vitamin D, zinc — some evidence for immune support; discuss with your provider
  • Nasal saline rinse — can reduce viral load in nasal passages and ease congestion
  • Monitor oxygen saturation — consider a $25 pulse oximeter from a pharmacy; alert if consistently below 94%
