- Nausea, vomiting, or stomach pain — more common in children, but adults get these too
- Fatigue — a heavy, knocked-down feeling
- Scarlet fever rash — a sandpapery red rash that starts on the neck or chest; not everyone gets this, but when it shows up, strep is very likely
Symptoms That Rule Out Strep Throat in Adults
Here is a simple filter: if you have a cough, runny nose, or congestion, it is probably not strep. Those are viral symptoms — the kind you get with a cold or flu.
Strep is bacterial. In addition, it generally does not cause upper respiratory symptoms like a cold does. For example, if your sore throat came with a stuffy nose and a cough that started at the same time, you are likely dealing with a virus instead. What Happens If You Don’t Treat Strep
Strep often doesn’t feel dangerous, so people wait it out. However, that’s a mistake. Without treatment, strep can cause serious problems:
- Peritonsillar abscess — a painful pocket of pus next to your tonsil
- Rheumatic fever — an inflammatory condition that can damage heart valves
- Kidney inflammation — a complication called post-streptococcal glomerulonephritis
- Spreading strep to others — you stay contagious for up to 3 weeks without antibiotics

Frequently Asked Questions About Strep Throat Symptoms in Adults
Can adults get strep without tonsils?
Yes. Strep infects the back of your throat, not just the tonsils. If your tonsils were removed, you can still get strep. You won’t see white patches on tonsils, but the other symptoms — sudden throat pain, fever, swollen lymph nodes — still apply.
Is strep contagious before symptoms start?
You become contagious roughly 12 to 24 hours before symptoms appear. Once you start antibiotics, the contagious period ends after about 24 to 48 hours. Because of this, staying home for the first day on antibiotics is a good rule of thumb.
How accurate is a home strep test?
Home rapid tests catch about 70 to 80% of strep cases. In other words, they miss roughly 20 to 30%. A negative home test doesn’t rule out strep. Therefore, if your symptoms are strong, get a clinical test at urgent care.
Can strep go away without antibiotics?
