Our model of care is rooted in creating solutions to break down barriers to access quality, accessible, and affordable care. CityHealth worksite health centers provide access to same-day in-person care with the option to engage with a virtual healthcare provider whenever and wherever. Our approach is centered around meeting the patient where they are at—work, home, or on the go.

Ensuring the highest quality of care is paramount. To achieve this, we involve and engage with our medical directors, providers, and support staff on a weekly basis to ensure that care is consistent, collaborative, and coordinated. Our Peer Review and Quality Control teams are consistently reviewing charts and providing feedback to the clinical teams to promote care standardization and continuous improvement.

We work to empower individuals to prioritize their health and guide them on their own personal journey to achieve their health goals. Wellness, nutrition, stress management, exercise, and behavioral health solutions are regular touchpoints during our onsite health visits. CityHealth puts the employee at the center of our model of care, surrounding each individual with a variety of health services to maximize engagement, develop strong relationships with our healthcare team, and help workforces and their families live healthier and longer lives.
